This work provides guidance on the care of patients with cigarette-related nicotine dependence. However, not all smokers meet DSV-IV criteria, some have comorbid psychiatric conditions which are not described in this work. Thus, the psychiatrist should not be limited to the treatments recommended.
This work provides guidance on the care of patients with nicotine dependence. Cigarette use is the most common cause of nicotine dependence, and almost all the data available is derived from cigarette users; thus the focus of this document is on cigarette smoking. The recommendations apply generally to all smokers, even though not all the smokers meet DSV-IV criteria for nicotine dependence. This is because most of the principles for treatment apply to non-nicotine-dependent smokers as well. It argues that many smokers have comorbid psychiatric conditions which are not described in this work. Thus, the psychiatrist caring for a patient who smokes should consider, but not be limited to, the treatments recommended in this work. This is intended for psychiatrists, but may also be of use to clinicians caring for nicotine-dependent patients.

Chapter 2 Disease definition, epidemiology, and the natural history: DSM-IV nicotine use disorders; specific features of diagnosis; epidemiology and natural history. Chapter 3 Treatment principles and alternatives: goals of treatment; assessment; psychiatric management; specific psychosocial treatments; somatic treatments; combined psychosocial and somatic therapy. Chapter 4 Formulation and implementation of a treatment plan: adult patients who are being seen by psychiatrists and who currently use tobacco; smokers who have failed initial treatment; treatment of smokers on smoke-free wards. Chapter 5 Clinical features influencing treatment: psychiatric disorders; concurrent general medical disorders; demographic and psychosocial variables; type of tobacco. Chapter 5 Research directions: epidemiology and natural history of nicotine dependence; epidemiology and natural history of smoking in psychiatric patients; mobidity from smoking in psychiatric patients; patient-treatment matching; treatment of smoking in patients with a current psychiatric disorder; treatment of withdrawal on smoke-free units. Chapter 6: Individuals and organizations that submitted comments.
