This sourcebook contains forms that a clinician needs to create accurate and complete clinical records. It contains blank forms which the reader can copy as well as completed copies of the forms illustrating the types of information which should be included. The accompanying computer disk contains all the blank forms in word processing form.
With the demands of third-party payers, as well as the need to comply with regulatory agencies in maintaining comprehensive treatment records, clinicians are faced with more paperwork than ever. Organized by stage of the treatment process (Intake; Assessment; Treatment Planning/Case Notes; Patient Handouts; and Discharge), this sourcebook contains forms a clinician may need in order to create accurate and complete clinical records. It contains both blank forms, as well as completed copies of the forms, illustrating the type of information which should be included. Also featured is an acompanying computer disk containing all of the blank forms in ready-to-use word processing files.
