It assists progress tracking by offering lab value spaces for five different dates on one form. The form provides a Body Mass Index table, as well as optimal and undesirable values of the following key metabolic variables in adults:

Total cholesterol, LDL, HDL, and triglyceride levels
Blood pressure
Fasting plasma glucose levels
Two-hour postload glucose levels
Random plasma glucose levels
The Metabolic Screening and Monitoring Form also provides diagnostic criteria and lab cutoff values for metabolic syndrome, prediabetes and diabetes mellitus.

John W. Newcomer, MD, is Professor of Psychiatry, Psychology and Medicine at Washington University School of Medicine in St. Louis. He is also the Medical Director for the Center for Clinical Studies at Washington University. Dr. Newcomer is a Principal Investigator on research grants funded through the National Institutes of Health (NIH), and also serves as the Chairman of the Drug Utilization Review (Medicaid) Board for the State of Missouri. Dr. Newcomer has received a number of honors and awards, including a 1999 Exemplary Psychiatrist Award from the National Alliance for the Mentally Ill (NAMI), an Independent Scientist Award from the National Institute of Mental Health (NIMH), and a 2002 Gerald L. Klerman Award for Outstanding Clinical Research from the National Alliance for Research on Schizophrenia and Depression (NARSAD). Funded continuously by the NIH for 14 years, Dr. Newcomer also serves as a reviewer for the NIH as well as other funding agencies, including the Department of Veteran’s Affairs and the National Science Foundation. Dr. Newcomer serves on the editorial board for Neuropsychopharmacology, Journal of Clinical Psychiatry and the Journal of Psychotic Disorders, and he is a reviewer for numerous journals, including the New England Journal of Medicine, American Journal of Physiology, American Journal of Psychiatry, Archives of General Psychiatry, and the Journal of Neuroscience. He has contributed numerous articles to leading scientific journals, including Archives of General Psychiatry, The Journal of Clinical Endocrinology & Metabolism, The Journal of Neuroscience, and Neuropsychopharmacology. Dan W. Haupt, MD is an Assistant Professor of Psychiatry at Washington University School of Medicine in St. Louis, and is an Assistant Director for the Barnes Jewish Hospital Outpatient Psychiatry Clinic. He is the recipient of a National Institute of Mental Health Career Development Award studying the metabolic effects of polypharmacy in schizophrenia. He is the recipient of two Young Investigator Awards from the National Alliance for Research on Schizophrenia and Depression (NARSAD), and was also named as a 2002 Lieber Investigator by NARSAD. In addition, he is a collaborator on other National Institutes of Health (NIH) funded studies of neuroendocrinology, including medication effects on glucose and lipid metabolism and adiposity. He is actively involved in teaching, supervision of residents and medical students, and clinical care. He has served as a reviewer for more than 25 journals in the fields of psychiatry, neuroscience, and endocrinology, including the Archives of General Psychiatry, the American Journal of Psychiatry, and the New England Journal of Medicine. He also serves as a grant reviewer for the NIH. He has contributed articles as an author and coauthor to leading scientific journals, including the Archives of General Psychiatry and Neuropsychopharmacology.
