The modern approach to the investigation and treatment of diseases of the liver relies upon close co-operation between hepatologist, surgeon and radiologist and each must be aware of the other's problems. In particular the surgeon should know to what extent radiologist can help in pre-operative diagnosis, the assessment of respectability, the choice of surgical procedure and the detection of postoperative complications. On the other hand, the radiologist must be aware of the needs of the surgeon and be able to provide a preoperative diagnosis which is as accurate as possible and to convey detailed anatomical information about the location of the lesion. In order to achieve the latter, radiologist and surgeon must speak a common language - the language of Couinaud and segmental liver anatomy. This book aims to present well-documented material on liver ultrasound in a succinct fashion and in a readable style. Using illustrations and case histories, it should be of practical use to radiologists, hepatologists and surgeons alike.
Part 1 Ultrasonography and liver anatomy: the appearance of the normal liver; liver parenchyma; normal liver size; intrahepatic vascular and biliary structures; ultrasonography and liver segments. Part 2 Ultrasonography and the diagnosis of space-occupying lesions of the liver: anechoic lesions; hyperechoic lesions; hypoechoic lesions; lesions of mixed echogenicity; hepatocellular carcinoma (hepatoma); adenoma and focal nodular hyperplasia. Part 3 Liver ultrasound and the hepatologist: cirrhosis; acute hepatitis; chronic hepatitis; fatty liver (steatosis); medical jaundice; the chance discovery of ultrasound lesions. Part 4 Liver ultrasound and the oncologist: the detection of liver oncologist: the detection of liver metastases; the investigation of liver metastases; monitoring the effiency of treatment. Part 5 Liver ultrasound and the intrahepatic biliary tree: dilation of the intrahepatic ducts; intrahepatic stones; diseases of the intrahepatic ducts; isolated ultrasound abnormalities of the bile duct wall. Part 6 Ultrasound and the liver vasculature: the intraparenchymal arteries; the intrahepatic portal venous system; the hepatic veins. Part 7 The surgeon and the ultrasound of liver tumours: the determination of the number, size and location of tumours; examination of the relationship between the tumour and the vascular and biliary trees; estimation of the volume of functional liver that will remain following the hepatectomy. Part 8 Interventional liver ultrasound: the technique of ultrasound-guided needle biopsy; fine-needle aspiration cytology and needle biopsy; percutaneous opacification of the biliary tree; percutaneous drainage of peri-and intrahepatic fluid collections; percutaneous alcoholozation of small hepatocellular carcinomas. Part 9 Intraoperative ultrasound and liver surgery: diagnostic aid; an aid to surgical tactics. Part 10 Ultrasound and liver transplantation: the technique of liver transplantation; selection of the donor liver to match the recipient; the use of ultrasound in the preoperative phase; detection of postoperative complications.
