Undersurface Arthroscopic Rotator Cuff Repair

We have modified a standard arthroscopic rotator cuff repair to visualize the repair entirely from the glenohumeral joint (undersurface repair). Our undersurface technique involves scoping the glenohumeral joint from a standard posterior portal, viewing the torn cuff from below, creating a lateral portal with a spinal needle, introducing a shaver under the cuff to debride it and the landing site, repairing the cuff with an inverted mattress sutures (ArthroCare Opus[registered] system in a single row configuration), and viewing the repair once again from under the rotator cuff.
