Distraction Arthroplasty of the Trapeziometacarpal Joint

This title focuses on such area as hand & wrist and joint replacement. Trapeziometacarpal distraction arthroplasty is a new surgical technique for the treatment of trapeziometacarpal arthrosis. The technique consists of distracting the first metacarpal and then using a tendon graft to anchor it in suspension to the second metacarpal. The result is a reduction of the subluxation of the base of the first metacarpal and an opening or distraction of the trapeziometacarpal joint with a clear decrease in the forces of attrition on its joint surfaces. Three years after undergoing the new technique, 20 patients (4 men, 16 women) (average age, 56 years) were evaluated using VAS scores, DASH scores, Grind tests, Kapandji tests, and radiological views. We also evaluated complications and patients satisfaction. Every patient reported a lower VAS score with a reduction of pain, and all patients expressed satisfied with the procedure. One patient, who had a fracture of 2nd metacarpal in the site of the drill hole, was treated with a plate. Radiographs showed a good reduction of subluxation of the trapeziometacarpal joint in all patients, and all patients returned to their normal every day activity.
