This book dives deep into the world of Gotham and analyses everything from Arthur's disorder, pharmaceuticals and the side effects it can have. What he writes in his notebook and what he is hallucinating about give's us and understanding about all of his negative thoughts and helps to understand his very dark humor that he is starting to accept on his evolution from Arthur to Joker.
