This text allows physical and occupational therapists to describe the relative abilities/difficulties of a patient in the four domains of impairment, disability, handicap and well-being, in order to monitor changes over time. Tested for reliability and clinical validity, this manual contains Core Scale Cards and Outcome Measures Client Detail Forms that should be simple to use. The practical approach allows for the gathering of outcome measures suitable for use across client groups by all health care professionals, and for the comparison of different services.
